What you will be asked
- Your email address first
- Your full name and phone number
- Your gender and age
- When you joined the church (month and year)
- Whether you have completed the Supernatural Life Institute, and if so when (month and year)

Certificate — Supernatural Life InstituteRequired if you have completed Supernatural Life Institute
Required if you have completed the Supernatural Life Institute. PDF, JPG or PNG. If you are still waiting for it, or cannot find it, choose that option instead.

Certificate(s) — other ministry/professional trainingOptional
Optional. Any other relevant ministry or professional training certificates. PDF, JPG or PNG.

Letter of introduction — requirements
Your letter must:
- Be addressed to the Selection Committee
- Be no fewer than 350 and no more than 500 words
- Preferably be no more than 2 pages
It should cover:
- Your salvation experience, including when you became born again
- Your experience of being filled with the Holy Ghost
- Your spiritual growth and current Christian disciplines
- Your membership and service within the ministry
- Your understanding of the ministry's message and vision
- Your reasons for seeking appointment as a lay minister
- Your availability and readiness to serve under pastoral oversight
